If you are a doctor and need to send a patient with radiographs or any other documents (images – PDFs) to our office, please complete the information requested, as well as a description of the concern. We will send you a consultation report by mail describing how the visit with your patient went, as well as a suggested treatment plan. If you would like to talk to Dr. Hassan before him starting treatment, please indicate on the referral. Thank you again for you confidence.
